• Honorable Dismissal / Transfer Credential
• 1 original and authenticated copy of the Transcript of Records / Course Program and Certification on Subjects Taken and Passed
• Certificate of Good Moral Character
• NSO copy or Local Civil Registrar authenticated copy of Birth Certificate
• Admission Form (Green Form)
• Four (4) copies 1.5" x 1.5" size picture in blue background and with name tag.
• Admission Slip
• Student Personal Record Form
• Medical Certificate (AdNU Form)
• Long white folder
