• Original Transcript of Records if transferee or degree-holder;
  Certificate of Employment, if working
• Certificate of Good Moral Character
• NSO copy or Local Civil Registrar authenticated copy of Birth Certificate
• Admission Form (Pink Form)
• Admission Fee
• Three (3) copies 1.5" x 1.5" size picture in blue background and with name tag.
• Admission Slip
• Medical Certificate (AdNU Form)
• Long white folder
